How we use PHI
We use and disclose PHI to provide treatment, obtain payment for services, and conduct healthcare operations — including coordinating your care with other providers, verifying your insurance benefits, and improving our services. We may also disclose PHI as required or permitted by law.
Your rights
You have the right to inspect and copy your record, request amendments, request an accounting of disclosures, request restrictions on certain uses and disclosures, and request confidential communications. You may file a complaint with us or the U.S. Department of Health and Human Services if you believe your rights have been violated.
