What a TMS Session Actually Feels Like, Start to Finish

The most common questions we hear before someone starts TMS aren't about the science. They're about the experience. Does it hurt? Am I awake? Can I drive after? Here's an honest, step-by-step walkthrough of what actually happens.

Before your first session: the mapping visit

Your first appointment is a little different from the rest. Before treatment begins, your clinician performs what's called motor threshold mapping — finding the precise location and the right pulse strength for your brain. Everyone's anatomy is slightly different, and this step is what makes TMS a targeted treatment rather than a one-size-fits-all one. Our clinic uses the Magstim® Horizon 3.0, which supports precise, consistent targeting from session to session.

Walking in

A typical treatment visit is simple: you check in, settle into a comfortable reclining chair, and the treatment coil is positioned against your head at your mapped location. You're fully awake the entire time. No sedation, no anesthesia, no IV. You can talk with your technician throughout.

What the pulses feel like

During stimulation, most people describe a tapping or knocking sensation on the scalp, along with a clicking sound from the device — which is why you'll wear earplugs or ear protection. In the first few sessions, the sensation can feel strange or mildly uncomfortable, and some people experience scalp tenderness or a mild headache afterward. For most, this fades noticeably within the first week as you get used to it.

What it is not: it is not painful in the way people fear, there is no electrical shock sensation, and you feel like yourself the entire time.

How long it takes

Sessions at our clinic run about 20 minutes. A standard course is five sessions per week for roughly six weeks. It's a real commitment — we won't pretend otherwise — but each individual visit fits inside a lunch break.

Immediately after

This is the part that surprises people most: when your session ends, you simply get up and go. Drive yourself. Return to work. Pick up your kids. There's no recovery period, no grogginess, and no restrictions on your day.

When people notice changes

Every person's course is different. Some patients notice shifts in the first couple of weeks; for others, change builds gradually across the full six weeks. Your care team tracks your symptoms throughout so that progress is measured, not guessed at.
